At your initial visit to our office, Lenz Family and Cosmetic Dentistry wants you to feel comfortable and welcome. After checking in, we will answer all of your questions. Following your appointment, any needed treatment will be discussed and fees will be explained, including your insurance benefits.

Please assist us by providing the following information at the time of your consultation:

• New patients forms fill out and signed. (Click here for forms)


• List of medications you are currently taking


• Bring any recent x-rays, within 6 months, from a previous dentist. You may bring them with you or request that they be sent to our office prior to your first visit.


• *** Please alert the office if you are allergic to any medications, such as penicillin, sulfa, latex, etc or if you need to be pre-medicated prior to your dental cleaning due to a joint replacement, heart murmur, etc.
